CE credits for eligible courses during the Buffalo Niagara Dental Meeting are submitted directly to NYSDA for members only. You must include your ADA number for automatic submission by the University. If you do not indicate your ADA number on the CDE Credit Verification Form, you must submit a copy of the form to NYSDA yourself.
Some courses may not be acceptable for New York State Mandatory Continuing Education Credit. Other organizations (AGD, ADHA, DANB, etc.) have different requirements for acceptable continuing education courses. Please submit copies of your CE credit form to those organizations when needed.
University at Buffalo, Continuing Dental Education, is an ADA CERP recognized provider. ADA CERP is a service of the American Dental Association to assist dental professionals in identifying quality providers of continuing dental education. ADA CERP does not approve or endorse individual courses or instructors, nor does it imply acceptance of credit hours by boards of dentistry. This continuing education activity has been planned and implemented in accordance with the standards of the ADA Continuing Education Recognition Program (ADA CERP) through joint efforts between SUNY at Buffalo, School of Dental Medicine and the UB Dental Alumni Association.
CE CREDIT VERIFICATION FORMS
1. ONE CE Verification form will be included in your dental meeting registration packet!
*** THIS ONE FORM IS FOR ALL LECTURES/COURSES YOU ATTEND THROUGHOUT THE MEETING ***
2. Complete the verification form as you attend lectures, including course titles, instructor's name, and verification numbers.
3. Submit the top copy of the CE verification form WHEN YOU COMPLETE YOUR LAST COURSE. Keep the bottom / carbon copy for you records. This is your only record of attendance! Collection boxes are located in the Convention Center Lobby. CE Verification forms must be submitted at the Convention Center and will not be accepted by the University after the meeting.

CANCELLATION POLICY
A cancellation fee of $25 will apply to all cancellations. Written requests for a refund must be received by September 20, 2012. No refunds will be issued after September 20, 2012.
PARKING SUGGESTIONS
Due to the limited number of parking spaces in downtown Buffalo, here are some suggestions:
1. To reserve a spot ahead of time go to www.allproparking.com and click on 'reserve on line' at the top.
2. Park at UB on Main St in the NFTA Lot ONLY and take the Metro Rail for a very low price.
3. Get downtown before 8am and park in the Main Place Mall, Fernbach or Ausburger Ramps.
